Im thinking inadequate lighting, water movement and even a feeding. I would look at adding another ligt/better light (and I know- expense) but will pay off not just for the nem but any future coral purchases. Focus on parameters, continued cycling of tank and equipment as you will then have a green light for all the pretty stuff later in which your tank will be Ready for them.
 
Just a quick update - I asked one of my friend to take care of my nem. I realized my tank isn't matured yet but his one fully matured and nearly 2 years old and a much bigger one. I'm gonna take all your advice and get him back or get a new nem may be in another 8-12 months :) until then i hope my clowns will be ok :D
