Dying leopard Wrasse?

Please need help , I woke up to my leopard wrasse being like this , he seems to be alive , he tries to swim but keeps going upside down. (Rolling )
Not sure what to do or what could be the problem .
I have pods and dose pods often(culturing my own)
He was eating and seemed fine yesterday
Testing water right now

Have you checked your water parameters including ammonia and salinity? Did you QT fish before adding it?

Yea I’m doing that now .
So far
Salinity 1.025
Temp: 78
0 ammonia
0 nitrite
5 nitrate
I did not quarantine didn’t have a quarantine tank at that time .
My lfs had him for a week before I decided to get him .

Oh no. This fish unfortunately has shallow breathing and appears to be moribund, and I hate to say but chances of recovery will be low. If fish was active yesterday, it can be crashing into object which often you can tell from appearance of lips, internal such as constipation or a spike if this is a newer tank.
What is temperature and salinity as well as ammonia levels?
What other foods besides pods are you feeding as this fish also needs brine and mysis shrimp as well as small plankton and even blackworms as their digestive tracts are short.

What is the tank size? I see in your other thread you have 20 gallon, this fish should be in 90+ gallon tank.

Yea I’ve been feeding a mix of brine and mysis as well, that was my though that he might of hit something specially when the cleaner shrimp tries to get on top

I had him for 4 days , yea so the idea was to have him here temporarily and them move him to the tank at my job (my mistake I know ) so a 65 gal tank is not big enough for this guys?
