Help ID angel fish

A coworker has an angel he wants to rehome to me. It was his daughters, who recently moved out, so he's looking to tear everything down. They have had it the past two years in a 25 gallon tank :-/ Yes, you read that right.

I'm not sure what kind it is. It almost looks like a Koran, but the white face is really throwing me.
Any ideas?

I'd like to read up on the kind, if we can figure it out, to figure out tank specifics. My tank is on the small end of the spectrum for angels. I am currently running a 90 gallon. Small for an angel, but definitely so much better than a 25 gallon.

Thoughts / opinions?

It looks as though that Angel has a horrible case of Lateral Line Disease, but I would need a better image.
 
Okay, so we definitely figured out it has had a rough go. I am not familiar with lateral line erosion. What's the cause? Best way to relieve it, if any? Contagious? Fatal? Does it cause any other issues?
Is it something I just need to pass on? Recommends a 200+ gallon tank, and Im running a 90...only thing is, if I do pass on it, it'll just stay in the 25 gallon :-/

Lateral Line Disease is often caused by nutritional deficiencies. Other cause can be poor water quality or stray voltage. Typically the condition is slowly rectified with proper diet..
Also consider these fish are sometime not reef safe and will eat corals.
I am not going to comment on tank size, but this fish gets huge.

I really hope it goes to a good home, it's really sad to see a fish in such bad condition.
