I have a qt tank all set up and have a 75 gal dt that has ich. I have been about 5 days into kick ich treatment with rally. It has helped some but the cycle is back and one fish is really sick. Should I move him to the qt now? He may die either way I guess.

Moving the fish now will most likely kill it if it's not already doomed. The less stress the better at this point. Sorry. :(

Yes, move the fish. Kick Ich is essentially water-in-a-bottle when it comes to treating Marine Ich. It does nothing. You will need to move the fish (all of them) and treat with Copper or Hyposalinity.

Any other treatment, you're just spinning your wheels and going no where.

Some people are not going to like me saying this but...

I have had a little success with a fish that had ich (I think it was ich but might of been battle wounds) in my display by soaking the food in Kent marine garlic extreme and a uv sterilizer. It was a powder blue that i had quarantined for two weeks in a copper treatment, dropped in the display and my yellow beat on him good. Two days later he had what looked like a few little bumps on him. I already have a uv sterilizer and just started soaking the food in the garlic extract to see if it would live up to the claims. After two weeks, the powder blue's skin was as smooth as my son's backside. I don't know why it worked but it did, I also feed two times a day with a mixture of veggies and carnivore diet. I am not claiming that it is a cure but it did work for me, from my understanding healthy fish build up an antibody to resist and fight ich, it is just a manner of keeping them healthy enough to fight it off.

I like your answer but they won't like mine. LOL! I have found the best cure for ICH is to feed well and give the fish the best possible water quality and stress free environment I can.
