Wow. That’s very low, IMO.

How are you mixing the new salt water?
I add Tap water Pittsburgh has pretty good water where I live. Live now in apartment for two years without previous problems, No Room or am i able to use ro Di water due to limited space and restrictions apartment policy. Stopped with coral tanks due to inability to prefilter water. For 2 years no problems with this method. I have always kept x 20 years FO tanks at lower salinities, FISH HAPPY eat like Pigs and No disease issues. Sump using reef octo skimmer space saver and chemipure elite or blue as needed. Only happens with water changes maybe bad batch of salt, awaiting new bags IO. Thanks
 
Never had a problem with Reef Crystals. During the Summer, I put 2/3 of the Salt in the RO/DI Water. Let it mix, then bring it up, usually with two more salt additions. One estimated at 1/3, then the fine tune. Winter when the Water is Colder, first salt is added at 1/2 the amount, then I bring it up with two doses. At Each stage it is throughly mixed for an hour or two.

Are you adding cold tap water directly to the tank?
 
Don’t think of tap water being consistent. Even though you’ve been doing it for many years, water companies do many things when supplying the water to your house. Sometimes they will change the source water or laws will change that require them to add or take away the chemicals used to process it.
