Snowflake Eel Not Eating

My snowflake eel hasn't eaten for three weeks, and I am getting worried. He lives by himself in a 75 gallon FOWLR tank. Other than nitrates being at 40 ppm, the water parameters are good. I've had the eel for nearly six years, and he has gone through periods of not eating before, but never as long as three weeks. I normally feed him squid, silversides, and clams.

He has a small black dent in the left side of his face that wasn't there before. Also, he's had a some discoloration (possibly an injury?) on his back for at least two months. It didn't affect his eating previously, but is it an issue? Should I try treating with Melafix or another medication?

Have you tried feeding it krill? How about some vitamins like Vitachem? That sore looks a bit rough. How long has it had the sore? If the sores increase or the situation becomes worse (like permanent fast breathing or spasms) consider a treatment with an antibiotic such as Maracyn II in a QT to help its immune system.
