what is wrong with my frogspawn???

Hello...the last couple days I noticed a couple pieces of my frogspawn floating around. Why would it do this. I have 3 and the they all "look" ok but I'm wondering if something is wrong. I have 3 frogspawns, 3 hammers, a few mushrooms and assorted zoas and palys. Ill do more searches but if anyone can help I would appreciate it.
 
I'm no pro.. I wonder myself what it is,, what kind of fish do you have? Is there a new addition maybe that could be doing this... Maybe it's some kind of disease i don't know of but then again I'm no pro and wish i could help more...
 
How long have you had the frogspawn? What are your water parameters? It could be that there is a fish or something picking at it. Could you post a pic of the frogspawn and the pieces?
 
Is it polyp-bailout? Where the entire head of the polyp leaves the skeleton or are you seeing random pieces of tentacle floating around?
Sometimes clowns will nip parts off of Euphyllias, but that only matters if you have clowns.
Are the corals fully extended during the lighting period?
Do you have values for your water chemistry including alkalinity, calcium and magnesium?
 
I appreciate the comments. I have 2 clowns that are hosting two bubble nems. 2 bangaii cardinals, lawnmower blenny, yellow tang and a flame hawkfish. I've had this frogspawn for about 4 months. Haven't added anything for 6 weeks. And so far I've seen 2small pieces (looked like one of the dots from an arm) they were both very small. Today I found a little bigger piece that had about 4 dots and a little of the arm). I didn't take pictures of them. If I find anymore ill get a picture. Yes it is fully extended when lights are on. I'll get a picture up tomorrow when I get home from work because lights are out now. I do not have all the tests you asked about. I planned on getting them this weekend. Everything else is where it should be except Nitrates which are 20 (they have continued to be 20-60). I've had the tank since begining of Jan 2013.

If you are just seeing the occassional piece I would not be concerned. I have experienced the issue it never resulted in anything significant.
 
Well good news I didn't find any pieces today when I got home. I guess I won't worry unless I see it acting weird. Here is a picture of it with lights on. Thanks to everyone who replied, I really appreciate your help.

We had the same problem, it was out True Perc Clownfish that were living in it. They were picking chunks off and other corals were getting stung. Thats why we traded them for our new Extreme Snowflakes.. No problems so far.
