What killed my fish?

My Ventralis Anthias died within 3 days of bringing it home from the LFS, where it had been healthy for the past 2 months. Since they are a delicate species, and it had done so well at the LFS, I skipped QT, and put it in an acclimation box in my DT. I acclimated it to my tank with a 15 minute open bag float followed by a 30 minute process of slowly adding tank water to the bag Day 1 was fine, it ate. Day 2 I noticed its scales beginning to lift on both sides around its belly. Day 3 its belly began to swell, and the area of lifting scales spread. At this point I put it in my QT with epsom salts (in case it was constipation), Prazipro (in case it was internal), and metroplex (in case it was bacterial). I skipped feeding day 3. Day 4 I found him dead with the lifted belly scales missing. Tank parameters (in both tanks) were 0 ammonia (badge and test), 1ppm NO3 (Red Sea), PH 8.3 (Salifert & Probe) See pics for progression.
