I've come to the conclusion that the tank was infested with velvet (see other post for more info) Here are the

symptoms my betta had:
•Red eyes
•Lethargy and staying in corner near top of water
•claped top fin
•tattered top fin
• small black dots coming on his top fin
•loss of appetite
•He slowly lowered onto the floor and then sat there
•Gold luster

My betta is now dead, along with the tetras. The otos and the shrimp appear to be fine, but I won't know until tomorow.

Please advise on what I should do.

Glad to help reef tank or not.

First what size is the tank. Next, you would need medication. The types of meds that exist treat bacterial, fungal, and viral infections. So before you medicate, you want to figure out what exactly is happening. The problem, its always kind of a guessing game as those symptoms can mean a lot of different things. Do you have pics of the fish?

Regardless, I would start by using API aquarium salt. Second, I would test the water (poor water quality reduces the slime coat of a fish and allows it to become more susceptible to disease). If the water is poor, a slight water change followed by appropriate medication would be in order. However, if your fish are really dropping fast, It might be too late.

I'd start by posting pics and water parameters.

@Flutt3rby3 I think you are posting in the wrong forum. Aquarium care center is a store- that is why you are not seeing many replies here. Post in tank emergency or something for quick help! #mods

Thank you for replying, I have ordered the aquarium salt so it will arrive tommorow. The otos are starting to show one of the symptoms, red eyes. The snowball shrimp ate a piece of the body of the cardinal, so if he dies (hopefully not) then we can rule out a few things. I have a local-ish vet that is open to treating fish, so I'm going to ask him to prescribe me treatment for the fish. But we're all gobsmacked. I can't really identify it. The size of the tank is 26 litres (about 6 gallons) and is fully cycled and established.

Here are some pictures, I partly think it could be a parasite because he was rubbing up on ornaments a lot. Like he was trying to dislodge something. :/

I've found the culprit, the bloodworms. They carried some pathogen so that anything that ate them died. I added a general tonic into the water that will kill any pathogens. (its main ingredients is methlyne blue along with something else) so after that clears up I will get another betta and learn from my mistakes. My water quality is perfect according to the water quality check although that will be because of all the plants. XD
