I don't know enough about corals, are these tentacles or is he dying???

Looks like sweeper tentacles. It’s just how they collect food.

As stated above, those are sweepers. Your acan is hungry. I target feed mine daily to every other day with frozen mysis. They really appreciate it and will grow much faster and stay fluffier if you do so.

I see fecal material being expelled in the one picture, but overall, that Acan looks pretty good, and I'm pretty sure I see sweepers on the one polyp. Hard to tell with the blue lighting though

How old is the tank? Parameters? Test kits used? What fish are in the system? And when did u get the acan? Hard to tell overall condition under blue lights. Mayb a filter or whites. And welcome

Maybe, if that's what you see, I see acans deflated and stretched thin to their skeleton, with what looks like mucus rising up from it. "Sweeper" tentacles on an acan are not that long, but I guess it up to interpretation with those pictures so who knows, but a happy acan should be big and puffy daddy

I'm with this, ^. Looks stressed and mucus coming off. It's deflated and the backside of the right head doesn't look right. Did it get nipped at, bumped, or something? Are the lights going down right at this time of pic?
Mine deflate at lights out but never mucus unless warfare or bumped or something.
It does have new heads coming in on the left, that's a good sign.
