acclimation box - what went wrong?

Hi all,
I had my carpenters flasher wrasse in qt for a month. Looked healthy, eating great.
Yesterday i but him in an acclimation box and added him to the DT. He swam around frantically for a minute, then hid in his tube and breathed very heavily all day. This morning he is dead.

No one showed aggression. he just hid and died. What should I do differently next time to avoid this?

the salinity was 33 vs 34. I can't measure ph on one tank. i acclimated in a 50/50 mix of water (from each tank) for about 20 minutes prior to putting him in.
 
That's not far off. IMO maybe the 20 minute mix may have stressed. I am not one to drip or mix acclimate. As long as salinity is close I usually just drop the fish in. I have had much better luck doing so.
