If you’ve got soft corals, probably quite often. I’d only use a little to clear any yellowing in other cases, personally. It’s a bit of trial and error to be honest.
 
Hopping on. I have no idea exactly when but do change it out if things change for the worse
 
I was told once a week but I'm stretching it out to two weeks at the moment. Skimming wet, diy chaetoreactor, all softies tank at the moment.

Depends on a number of factors.

Size of the system
A bag in the sump is different to passing though a reactor.
How much you use
The amount of DOC in the aquarium.
Stocking (Softies vis LPS/SPS)

For my 90 gal system I use about 100ml in a reactor and change out every 4 weeks.
 
On my softie tank which is full of leathers and other soft corals, I change it every 2 weeks.

On my main tank which is lps and sps, I change it out monthly.

I stuff the bag in my nylon filtersock.
