I lost the Flame Angel in this pic during new arrival QT. It survived six days from arrival before succumbing, and I was getting my hopes up :(

It arrived with what I was sure was ich visible mostly near, and on the tail fin. I use TTM anyway so that was in work. By the time of death all signs of that, anyway, were gone. There was one tiny white spot on the forehead that didn't look like ich, but some kind of growth even though still very small.

What was done: The fish was also flashing and gulping at the surface so the 1st tank was cut to only 24 hours, formalin bath at the end, and into second tank. Second tank was close to the 72 hrs and a round of Prazi. It was in the 4th tank about 48 hours when the small white spot seemed to go out of control overnight. I wanted to get it on some antibiotics quickly so it got another Formalin bath on the way out to the 5th tank where I started it on Furan-2. However, it didn't make it through the night :( Also, its fins were ragged by the day it was moved and Furan started, and by morning the tail fin almost gone.

So, can anyone better confirm what this is? I'm only assuming fungal or bacterial. What better should have been done? Antibiotics sooner I think, although it didn't appear significant for awhile so I intended to wait until the end of the 12 days to start until it exploded almost overnight. Second, there is a visible hole in the center. Did Formalin on an open sore likely become a huge factor in the death, even if not the root cause of the issue?

That looks bacterial to me.

Flame angels gasp for air quickly it's a stress response and also because they truly have little tolerance for even slightly low oxygen conditions, in my experience.

It could have been a mix of things, it's hard to tell at this point but the head looks like an injury and possibly infection.
 
I also agree with bacterial infection. It's hard to say though whether he had ich or velvet without knowing more, timeframe seems more like ich. Although with how rampant velvet is nowadays I have pretty much given up using TTM and go straight for copper.
 
Thanks 4FordFamily, at least it confirms likely bacterial. Having taken a break from the hobby I don't trust myself, lol. Still bummed I couldn't save it though :( It's my favorite fish and was excited to have one working its way to the DT!

GHsaltie, thanks as well. As I said, I took a break, but I "think" it was ich anyway. While it was only around the tail each spot looked too large to be velvet. However, it doesn't mean he didn't have velvet as well or my time away skewed what I thought I was seeing! 270g DT new build to get stocked between work travel and now another setback :( Well, I guess if the glass is half full I just prevented a catastrophe, lol.
