Algae outbreak need help!!!!!!

I've had an algae outbreak for about a month, I have tried everything I can think of! I left lights off and covered the tank for 3 days and nothing. I have carbon in and I'm dosing vibrant and weekly water changes. Just as it looks to get better it comes back even worse. If you can ID the algae and suggest anything I'd appreciate it!

What are your parameters (po4, no3 specifically)?
Can you get a white plastic lid or plate, add a little tank water on it and then take some pieces carefully from the tank of the different algae and place them on the plate and take a picture. So we can get a better look at what you have?
 
I would also like you to do one more thing after that. On that very same plate or lid add a squirt of hydrogen peroxide to it. Let that sit for several hours (overnight is good) and then take another picture.
