Anemone dying?

My anemone decided to move off the rock and move to the wall. Now it looks like this? Is it dying? I'm worried it might detach and suck into the power head eventhough they have filters on them.

It doesn't look all that healthy. How long have you had him and how seasoned is your tank? Got any parameters that you can share?
 
Testing my water right now. Had it about 3 weeks. Tanks been up about 6 or 7 months
 
Will wait to see what the testing reveals but, that is a bit of a "new" tank for supporting an anemone. What lights do you have on your tank? How large is your tank?

Temp 78.4
Ph 8
Nitrate 30
Nitrite .05
Ammonia 0
Phosphate .5

Nitrate and phosphate are higher than normal and I plan on doing a water change

How should I do that? It will poison the tank right?

It probably won’t fight you much if you just pull it off or put something behind it and get the foot unstuck. It’s most likely very weak. Just do it fast.
