Anemone Not Doing Well

Hey guys, so I got an anemone from a guy, believe a bubble tip, about 5 months ago. The anemone was very large and healthy looking when I got him. About 3 months ago he suddenly lost all color and no longer comes out. He never moved from the spot he picked which is at the very bottom of the tank. I changed nothing about the aquarium idk what brought this on. He is still worsening and don't know whats wrong. I have duncans, frogspawns, ricordea, and zoas all in the tank and all doing well. Only thing I can think of would be lighting. I have a AI Prime HD setup 12 inches above a 32 Biocube.
Setting on AI are as follows:
Watts: 24
Uv: 15%
Violet: 80%
Royal: 90%
Blue: 80%
Green: 25%
Red: 0%
Cool White: 15%

Water Parameters:
Temperature: 78 degrees F
Ph: 8.4
Ammonia: 0
Nitrite: 0
Nitrate: 0
Dkh: 8
Phosphate: 0.5 (currently small algae outbreak)
Calcium: 420
Salinity: 1.024

I'm not an anemone expert by any means, but if he is dying, you might want to consider removing him before he completely goes. If they die and are not taken out quick enough they can cause a tank to crash. For the sake of your other livestock I would be really careful. Good luck.
 
P.S, if your water starts getting cloudy, it means its dead. Do a big water change and remove any pieces you can find.
