Anemone on the verge.....

I've had this anemone about 4 months and about 2 weeks ago it started to move around and appears to be slowly deteriorating after some good growth. I guess it's to the point where I'm looking at it wondering if visually it appears to have any hope? I keep thinking about removing it...but you can tell there is still life there yet. It's in a 24 gallon aquarium so trying to make sure no nutrient release upon death...if that's the case.

Yeah, it's not looking good. Maybe post in the clownfish/anemone forum to see if you can get some better info.
 
What kind of anemone is it supposed to be?

It doesn't look happy. I'd consider pulling it out. I breed some anemones and collect some in the wild. I often keep them in a separate container, like a 16 oz. deli container that pineapple rings come in from the grocery store. I'll put a little rubble in the bottom, but they typically want to set their foot on the plastic. Then every morning and every evening I'll do a 75% to 100% water change. I'll dump the old water down the sink and scoop up new water from the tank. I've kept anemones in this way for weeks while I watch to see that they are healthy and safe to put in my tank. I also do with with baby rock flower anemones which are so small (smaller than this asterisk *) that they are very easy to lose even in a 50g cube!

It is a green quadricolor. Appreciate the tips too. You have some interesting approaches mentioned there. Clams and anemones are probably my main draw to reef tanks and I'm trying to understand them more.
