How long has it been set up?
What are the current water parameters --- with numbers please.

What all is in the tank -- fish and invert wise.
How long has it looked like this?
Overall, for an H. crispa, it doesn't look too bad, decent color, and average tentacle length. Just looks a bit "unhappy" at the moment.
