Anemone was doing fine. Now it's going down hill.

I bought an bubble tip anemone a few months back. It was bleached and barely had any tentacles when I got it. Within 3 months it grew its tentacles back, gained a lot of color and grew in size. About 9 days ago it shrank to about the size of a golf ball and drew all it's tentacles in. Yesterday it detached from the rock and fell in the sand. I tried to put it on a rock and it stuck to my finger, but refuses to stick on the rock again. I've also seen a huge diatom bloom that started about the same time. My ammonia is at 0, nitrite is 0 and nitrates close to 5. My sulfate is at around .25. Does anyone have any advice or ideas what is going on? The color of the anemone is still vivid and is still moving on its own.

How old is your setup? Anenome's require a healthy and established tank of around 10-12 months old. Your Parameters listed give some information but not enough to get the whole picture. What is your Calcium, Alkalinity, Ph and salinity at? Any more information on your tank is much appreciated to get your anenome back on track.

My set up is about 7 months. Parameters have been steady since i added dr tims one and only about 5 months ago. Ph is at 8, calcium is at 380 and salinity is .026. I also have a viparspectra 165. Ch1 is 21 and ch 2 is 9
