Thanks for the reply,

Temp - 26 degrees
PH - 8.3
Alk - 10dkh
nitrate - 2.5ppm
phosphate 0ppm
Ca2 - 380 mg/L

those params have been stable for quite some time before that rock with anemone was added, with all other fish and coral having no issues and growing fine.

It looks like the mouth is right by the edge. Maybe it is from a bad split or it was torn when the rock was removed from the old tank. Those are best guesses.

What test kits do you use

Is it best to just leave as it is or anything that can be done?

0.00ppm is not good for phosphate. You want a little bit.

If you want to, try adding some brand of dissolved foods (reef energy, fuel, aquaforest af energy, vitachem). I like these for anemones that aren't doing well. However, I think the anemone will be ok as long as it's attached to the rock. Once it can't attach then I would be more concerned

It looks fine. Did the tank the rock was from have rainbow bubbletip anemones? Looks like a baby bta from what I can see.
