Not unusual especially where there is insufficient light and/or current. They need light to produce zooxanthellae which provides it color and energy. Too much water flow will cause them to "Pout" in a fashion.

Should I put more flow? Its sitting at the bottom of my tank

250gph pump on a spin stream nozzle and a hydor nano 240
 
Is it dying or no? It looks like its dying eith a weird hole thing on its mouth that doesn't look normal

Post your parameters and by the look of all that algae, there are issues. Also how long has the tank been set up?
 
My parameters are fine, I'm freaking out
What are your parameters? Telling us they’re ‘fine’ doesn’t tell us anything. How big is the tank? What kind of lighting? How old is the tank? What kind of rock do you have? Tankmates? Need more info to give you a direction to go in.
