Got this Bicolor angel on Monday of this week

and last night eating and fine. Today twitching this morning and hiding, then tonight gone. This is the way she looks now, some problem with the area above the mouth. In the QT tank with her are 2 clowns but we saw no fighting or problems. The tank is a 30 gal cube with 30lbs of rock and only a small amount of Nitrate 20ppm, Sal 1.025, and no ammonia or nitrite. Any idea what this could be?

20 PPM nitrates is not a small amount.

As for the fish, that is weird, I am not sure what that is.
 
20 ppm is nothing for a fowler tank...it's possible he died And that's where bristle worms first started
 
pretty small tank for a bi-color and they are not all that hardy anyway around 50% never make it no matter what you do
 
Yes the 30 gal is small but make a good QT before the go in my 125 DT The survival rate is low but wanted to try and that will be the last time.
 
Flukes or oodinium would be my guess.
 
The open areas are probably from any scavengers eating (including the clowns). May be on the live rock. Without a necropsy it is hard to say. No signs of obvious parasites, but many do not show outside until too close to death to save them. Sorry for your loss. Good you are QTing.
