Animals Melting away

And the problems just continue. Ever since I went from a 45 to a 75 gallon there as been issues.

Now it seems some of my softies are "Melting" away. About two weeks ago my hammer literally melted away. I was horrible. In about two days was gone. Then some large Hairy Mushrooms that was on the same rock did the same. So I took the whole rock out of the tank. Now it seems that some more mushrooms on the other side of the tank are beginning to melt.

What the heck??

Please, any help or advise would be great.
 
Sorry for your problems.

What are your parameters? Is the new tank fully cycled? Any new fish or coral added?

Lights? Can you post pics?
 
Check phosphates, I'll reckon it may have to do with that unless there's like super whack water param swings
 
I did change from a 45 gallon 23 1/4 tall tank to a standard 75 gallon tank 21 inches tall. I used almost all the water from the 45 when I made the move. My parameters are 0 ammonia, 0 nitrates, salinity 1.023, Cal 525, Phosphates .25. Not sure on the Alk. I wasn't sure my test were right to I took a water sample to the LFS to test and they came back great.
