My angle has been doing great but today I noticed the eyes look cloudy. Picture 1 was 4 days ago and picture 2 is tonight. Seems to be grazing and eating just fine. But I’m concerned obviously all the other fish are healthy and no other signs of problems in the tank. Am I worried about nothing? I tested my water tonight and everything seems to be within parameters that I normally keep.

Beautiful fish!
I’d prepare a Qt tank for treatment once Jay chimes in.
You can use a Brute bin if you don’t have an extra tank. (Heater, air stone...).

Just an update medicated the 220 gallon tank with Melafix last night as prescribed on the bottle… woke up to a tank that had no problems for 8 years to this… heart broken only was able to save one of my monos

I should add that all of the ammonia/nitrates/nitrites/ Ph Where exactly the same. Only thing I can guess is that the oxygen came out of the tank

Sorry for your losses. I’ve been there and it’s heart breaking. Did you hear back from jay before dosing your DT? He would have suggested pulling the annualaris and treating in Qt tank especially if he was only one displaying symptoms.
Cloudy eyes can be signs of fluke or bacterial and having him I’m Qt would give you a chance to treat for both. Prazi for flukes and a broad range gram negative combo of furan2 AND kanaplex for bacteria. I’ve never used melifix and haven’t seen it prescribed by others.
