I have a male lyretail anthias in QT, first two days he looked fine, eating, very responsive. Yesterday I got home and noticed he had a spot on his head that scales were raised and a similar spot running from his anus up his side.
Yesterday he was acting fine.

I did a reef rally bath, started to treat with metro and kanaplex.

This morning, the raised scales have gone down a bit, but around his anus is very red with red in the area. It does not appear to be on the skin, but under the skin. Today he's being reclusive and doesn't want to eat. I tried to get pics, but he's hiding and I don't want to stress by forcing him out.

There's also a very small marine Betta in QT with him that has so far shown no signs of illness.

Should I be doing anything else?

Photos? Anthias are quite prone to uronema, but I’d like to see the fish first. It’s true that ich can display and dots that come and to, often within a 2-5 day period.

The anthias died, pics are attached. This progressed VERY fast, as he was fine yesterday morning. None of the red is an open wound or sore (I can see scales over it). It all looks like internal bleeding, especially around the anus.

My biggest concern at this point is what did he have and what risk the marine betta is at, having been in QT with him.

Sorry for the loss. looks like uronema to me.
That's what I fear, and I know that's what the pics look like. But it also looks like massive internal bleeding, as there's no wounds or ulcers on the body.

Assuming worse case scenario, what do I do with the marine betta, and how do I get him clear for the DT, assuming he continues to show no symptoms?

If you can get CP, thats probably going to be your best bet. If not, 14 days with Metro at double the dose every 48 hours should hopefully eliminate it.
I believe you can use Hydrogen Peroxide as well, but I'm not that familiar with it.

This, sorry for your loss. I agree with uronema diagnosis.

Appreciate the responses. I've seen that marine bettas's are very resistant to most Illness, is that also usually true with uronema? I'm hesitant to use copper unless I absolutely have to, especially given that it's an appetite suppressant and getting this guy to eat isn't easy.

On a side note, I'm done with anthias. The three that previously made it to my DT are the only ones I'll ever have. I've had more anthias die in the last month than all my deaths combined in the last year. 3 out of 8 alive is waaaaay to high of a death toll for my taste.

Yeah. I just cant handle that type of loss rate. Dead low for me in the hobby to be honest. I had multiple people tell me to try them, so I did, and I wish I'd never went down this anthias path to start, and sort of wish I hadn't added any to my DT already. These guys are far too sick and far too expensive to be worth it.

Yes they’re hardy but they can and very often do carry disease to other tank mates that aren’t as hardy.

So do you think the 14 days of metro will do the trick, to keep him from carrying it over? Or do you think something more intense is neccisary?
for uronema, 14 days in metro, and 14 days eating food with metro in it (uronema is internal as well). Be sure to add seachem "focus" to keep the meds bound to the food.
