Nearly finished with Qt. Copper power at 2.49 when I checked.
Nitrates 20
Phosphates 1.5

she has been fine for weeks and this started today. What do I do?

In Quarantine tank? New Fish? My first thoughts are possible cyanide poisoning if used to capture or stray electric current from PH ... but that would affect the second fish I see there as well. There are a LOT more people here with more experience that can hopefully help with good advice.

Yes this is a QT there’s 4 fish in there all went in at the same time. None of the others are affected. The QT started at therapeutic copper on 8/28

@Humblefish

he has some experience with qt tanks. Also looks like a possible swim bladder issue to me
I had him for about 2 months before I started QT. He came from LFS, was in my DT for 2 months, I pulled them all out and started QT. How long do swim bladder issues take to manifest?

Normally a parasite gets into them and weakens their slime coat which is their first defense. Then things start to go down hill as they can get viruses or get infected. Anthias are pretty delicate so if you dose meds do it sparingly. Water change him as much as Is possibly convenient and use something like selcon and growth hack to help him out.

I had a Dispar Anthias that made it through a 2 week QT without any issues, but then once I put it in the DT, it displayed the exact same symptoms. I assumed it was some infection with the swim bladder and I tried to catch it so I could at least isolate and hand feed, but I couldn't catch it. I kept waiting for the condition to get worse so I could catch it, but it never got any worse, and then eventually within a week was completely healed. Still going strong a year later.

I am far from an expert on fish disease, but at minimum I wouldn't do anything crazy because it is possible it will be able to recover on its own like mine did.
