Here’s a pic about 10mm it’s before this fish died. At surface gasping for air, trying to literally jump out the water. Blotchy / darkened. Any ideas ?

Aquarium type: just fish at present and some shrimp / snails
Quarantine tank : no quarantine but bought all livestock from same shop
Aquarium water volume : probably about 1200l uk
Filtration type : sump with skimmer / refuge / filter socks etc
Lighting: No lighting
How long has the aquarium been established? 3/4 months

Trying to jump out is generally associated with low oxygen, velvet or even aggression from another fish.
Take a look at ammonia-temperature-salinity levels to see if elevated

What's your flow/powerhead situation? Can you post a full tank shot?

Just off the cuff, if the onset of symptoms was very rapid, I would say Amyloodinium/velvet. If it was more gradual onset, like over a week or so, I would say flukes.

What fish remain and can you post a short video of them?

I would agree with @vetteguy - most likely oxygen or IMHO - ammonia - the tank looks pretty new. Also - if disease - velvet seems most likely.

Let me just give some background, had no signs of anything. Then I noticed my Freckle faced tang had a layer of “something”. At this time there were No noticeable signs of anything on any other fish. There is NO aggression in my tank. Then I noticed a bit of flicking and a bit of coughing/ head shaking. No cloudy eyes at all. I took the tank apart and fresh water dipped ALL Fish for between 4 & 5 minutes. My Blackcap gramma didn’t make it once put back on saltwater ( I think the salinity shock ) killed him/her. My other fish all came through it ok .A day later my Tomini Tang died in my display tank.

Scribbled Anthias
Peacock wrasse
Yellow tail tamarin wrasse

I had a cleaner wrasse that was acting weird - swimmming at the surface in the flow etc. he jumped out and was found later

The gills of fish deteriorate very rapidly after death. The ones in the picture don't look too bad, a bit less red that then should be. That is either post-mortem, or anemia, depending on how long the fish had been dead for.

Here are some clues that I gleaned from your post: the head shaking and flicking can be a sign of flukes. the blackcap dying after the dip can also be a sign of flukes, in severe cases, the fish simply bleed out when the dip knocks the flukes off.

You won't see dead flukes in the dip water (except one large species) unless you use a microscope. However, once dipped, the fish should improve after a day or so. The flukes will come back, but the FW dip should give them all a break.

However, I still can't rule out Amyloodinium/Velvet. A video would help with that.

With Velvet, the fish will all be breathing fast (but that is subjective, and many people miss it until the breathing gets really rapid). The fish loss will be rapid in onset. With flukes, the losses are more spread out over time.
