Any ideas what is wrong with this guy

i just noticed one of my chomis is not looking good. Any ideas what’s up? Had it a few years now and all of the other fish are okay

Yes, agree with above. Could be a number of things. Least bad would probably be constipation.
If so and it's eating and swimming ok, feed it a peeled boiled green pea. Supposedly that may get things moving. Never tried it myself.
If not swimming ok it may be swim bladder disease. Not as good of a prognosis.
 
Can you upload a pic in all whites? Looks like the stomach is extremely bloated.

I’m going to go out on a limb here but since it’s an older fish could it be eggbound?
I have no experience with breeding but do you have others in the tank?

Yikes, there was a post recently with a foxface that looked similar. Same recommendations were made, constipation, intestinal parasites, etc.

Apparently, it resolved on its own for some time and then randomly came back? I don’t think we really knew for sure what caused it.

I’d try the boiled pea and hope for the best... Feeding food soaked in Epsom salt, or dosing Epsom salt to the tank directly if he’s not eating, was another recommendation, for it’s laxative effects.
 
If it doesn't not end up being spawn. I may go this route as the fish appears bloated, scales stick out in a pinecone-like manner, symptoms of Dropsy. Typically found more in freshwater species.
