Have those splotches cleared up?

If not, I suspect he has a gram negative bacterial infection and those are not just stress spots.

I would give him a 90 min acriflavine bath: https://www.reef2reef.com/threads/acriflavine.282887/

And then into QT and treat with the following antibiotics: Furan-2 + Kanaplex + Metroplex

Agree 100%... That looks pretty bad, so act as quickly as you can for him.
