May I ask how long the tank has been up and running and the type of flow and lighting you use, please?
 
Have you checked for stray voltage? I just had something very similar happen to my nem and discovered a small amount of stray voltage from a small pump.

Tank age and parameters would be my 2 guesses. If both of those are solid then I would say infection and a round of cipro. Need more information.
 
Fish aren't grounded. Stray voltage does not hurt them.
 
What has it had to eat lately? I once feed nem some Fresh shrimp from grocery store, that must of had something to preserve it freshness. nem did not live long after that.

Fish have sensors on their bodies, such as along the lateral line and on their noses, which are sensitive to changes in electricity in the water around them. When these sensors receive a continuous supply of abnormally high electricity, it throws their systems out of kilter. An example of how this has been put to our advantage is in the "shark repelling" devices for swimmers and surfers that have been developed.

Now, nems are not fish, but in my experience invertebrates can be extremely finicky. Anecdotally, after removing the pump that was adding stray voltage, my nem returned to its usual fluffy self.

Something bad to eat would make sense.... I saw on here that someone fed their nem raw chicken at some point (yeeeesh)

Yes, like Rtaylor said, multimeter. I got one on amazon for ~$12.... there were also a couple other indicators for me that stray voltage may be the culprit... when my hand was in the tank I felt stinging and I was observing rapid jerky movements from my clowns.

Nems don't need food. If in distress feeding will make it worse.
 
Several weeks, and its a 10w HO-LED light
This is not going to work for an anemone. Minimum scenario with new nem owners is an established tank preferably over 6 months, strong lighting and well managed parameters.
 
Just the occasional fish to snack on... :rolling-on-the-floor-laughing:
Been lucky so far. My tang likes playing in the powerhead directly above the nems. Unsure how he hasn't been stung before.
