That's after a FW dip. Fish pics:

Flukes after a FW dip look like white-ish sesame seeds. Those look too small from the pictures.

Could be Brook parasites as they do respond to a FW dip pretty good. Or maybe velvet. Early Brook and velvet can present the same.

How's the fish's breathing? Any hiding from light, swimming into a flow?

Thanks Big G. They are smaller than sesame seeds. Background: this rared up week three into hypo @1.012, now coming out of hypo and at 1.015. Breathing is ok - not like a fish with oodinium. No hiding from light. No swimming into flow.

Also just noticed - the stuff after the dip is probably sand. It feels like sand and to catch the fish out of the tank involved catching him with a new at the bottom and getting some sand inside the net.

By the image it looks like either velvet or brooklynella.
