Thank you. For carbon I use chemi-pure blue. I’m actually over filtering. Water source has been from the tap but just switched over to RODI.Can be due to:

I’ve got two power heads and for agitation.It happens on newer tanks. The important thing is to make sure there is plenty of surface agitation so the oxygen levels in the tank don't drop too low. I had all of the fish in my tank die within a few hours once and I attribute it to low oxygen.
