judging by the picture it looks like a bristleworms but the wrasse actually eats those, so i am unsure of the cause of death. could be old age? water chemistry?

Im thinking old age

even though someone else lit me up the other day saying no

It looks like something ate out the fish's belly - but that likely happened post-mortem. Bristleworms can certainly act as active scavengers, but they won't cause the original issue.

The skin discoloration and lips are odd, not sure what to make of that though. Are all other fish symptom free?

Yes-all other fish seem symptom free

I checked online and it states that those wrasse live to about 10 years. I think that’s pretty good and definitely above average. Were all the fish quarantined?

No

i live on the edge

in my 15 years of reefing have never quarantined

had the great depression that happened once from a blue tang about 12 years ago

otherwise no issues

I think the fish died from old age. I’m not seeing any lead on parasites unless flukes were weakening the immune system. Have you ever seen scratching on sand/rocks?
