Been searching! Yellow worm?

So I've been searching the forums and just in general. My Aquarium is probably 8 months old now let it set empty for a while noticed some flatworms narrowed it down to probably white planaria....also noticed a bunch of very small Yellow worm like things on rock and glass with little tentacles/strings. Any suggestion on what they are. Moved a bit of my GSP from another tank into this one and it's super un happy all my levels are fine and light is sufficient and flow. Only thing I can think of is either the flat worms or these little yellow things aggravating it.....

Nope closest thing j could find was a spaghetti worm but they don't really look like that too me so trying to figure it out but my gsp in that tank is not doing well at all and it sucks

Tube worm would be my guess
