Bella Goby Issues

My bella goby who I have had for about a year has had no issues until the past week when it seemed like he lost weight, I than noticed 3 bumps near the rear of him on each side, I thought maybe these were part of his body and the weight loss was making these more visible. Hes been sifting the sand fine, and I was assuming he was getting enough nutrition from that and daily feeding. I started target feeding him twice a day and he seems to be eating some of the mysis here and there at the top of the tank. Today I noticed one of the bumps seems to have something coming out of it and I think it may be a sickness of some sort. No other fish are showing any symptoms and seem to be happy.

I have a 20long I can setup today and quarantine if this is needed, My only concern is if he is a sifter will the lack of sand in a qt tank negatively affect him.

Can someone first help me identify what is going on I looked at the stickied posts and Im still unable to identify the issue.

Goby looks very thin. Probably internal pathogens plus some sort of bacterial infection setting in on the fish's skin and maybe setting in on the internal as well.

Consider treating in the 20 QT. It's ok to put some sand in there. And you can use some from the DT.
Feed the goby General Cure + Food + Focus to send the meds directly to the gut. And then also treat the QT water with an antibiotic like kanaplex or Furan2 for 10-14 days.
