Looks like an infection below the surface of the skin that's starting to propagate. Possibly Uronema. I have to be honest, when I see a fish that's that progressed with an internal type infection it is very hard to pull out a successful treatment in my experience. If it's eating well that's a great start, but you'll have to give metronidazole treatment and formalin baths a shot to start with. Maybe someone else can chime in with a proven protocol, but I QT a lot of fish and those kind of infection sites are some of my most feared signs of disease. Usually not something that spreads to other fish in the QT in my experience, but definitely really tough to save a fish that is already affected. Best of luck!
