First, Welcome to R2R :)

I haven’t had a beta in a long time. How has it been prior to this - eating well, active?

This is a saltwater forum, but I’m sure we’ve got a few folks with betas that can help.

It's been 4 months and I have it and treated water with chlorine

How to check that

Test kits available at the fish store where you bought it.
Get fresh water, remove chlorine and transfer over, do you have an air bubbler.
Maybe low oxygen too?

Yes I have air bubbler you want me to add

Okay I'll do it right now.

small movement in the water is good, air bubbler should do that. how big is this betta tank? I am hoping he is alone, sometimes you can add 1 tetra fish in there if its a bigger tank just to give them a companion

Anything else required one of my local person told me to add stress reducer conditioner .

He is alone added air bubbler also. I have added some antibiotics powder that's why it's yellow.
