How long have you had it? Are all other fish in the tank doing ok? Was there any aggression? Wounds or spots on the fish?

Unfortunately, it’s usually too late when that happens. I’ve had very few fish come back. I know some folks try a bunch of stuff but IME it stresses the fish more. Perhaps best to isolate in a small tank and dim the lights.

we just checked our levels and ammonia phosphates and nitrates were all low close to zero.

Just to clarify, your ammonia level needs to be undetectable. Even small traces of ammonia in the water are harmful to all aquatic animals.

Based on the photo, his only hope might be a 30 min Methylene Blue bath: https://www.reef2reef.com/threads/methylene-blue.257467/

But euthanasia is probably a more realistic/humane option at this point: https://www.reef2reef.com/threads/how-to-euthanize-a-fish.266222/

I'm sorry :(

hes not too old yet dont know exactly how long weve had him. and there were no signs of aggression in the tank. we just checked our levels and ammonia phosphates and nitrates were all low close to zero.

Phosphates and nitrates would have no effect on the fish. Is the fish eating? What test kits are you testing with? Nitrites turn into ammonina but that does not seem to be the issue. This fish still has color.

The only thing is would suggest is to also add vitamin C and M for stress along with the recommenced treatment.

The scales almost look raised, is that an accurate characterization? Sorry for your loss, and I think that’s what this will be.

How long has the tank been set up?

Thank you all for your concern and suggestions, unfortunately Ernie the Bi Angel passed yesterday afternoon.
