My wrasse has took blister like spots on its nose and pigment has gone from around eyes. I have had him over a year and he is still eating really well and still very active but really not sure what this marks and blister like spots are

This is both a bacterial infection and head/facial erosion (HLLE) and Is likely due to both water quality issues and diet
What is your current ammonia-nitrate-ph-salinity ?
What foods are you feeding?
Treament for bacteria will be seachem Kanaplex in a separate quarantine setting and even adding Seachem Metroplex at .5gm weight (tiny dab) to the foods.

Thank u for reply. They get feed white shrimp, shrimp plus dry food also normally Mysis but shop was out of it . Only level which is bad is my nitrate which I have started today using hr nitrate remover. Not too sure if my issue is that I use water from the sea, I do 50 litre change a week and it’s a 300 litre tank. I have been using sea water for well over a year due to me constantly losing fish when I was buying the salt water, I lost my whole tank about 4 times and since using sea water I haven’t lost any fish.

It looks like HLLE. There are numerous purported causes - which are easy to look up. Sorry - I'm assuming - he is otherwise acting/behaving normally? Are you using a carbon? (not carbon dosing/but - activated carbon). I personally do not see the bacterial infection

If you are not taking your 'sea-water' from far from the coast - unless it's a reef environment - that may be your issue. I assume you are taking water from far out from shore. HLLE is a difficult issue - because there seem to be so many people with so many reasons for why its happening

He is acting totally fine, I have carbon in with my skimmer and pump

Not sure if this species is susceptible to LLE, but I do know that it's common for them to develop tumors in their snouts. These tumors are usually benign but can continue to grow over time.

Yes and make sure the carbon is not tumbling and is in a static location where water moves through it but does not move the pieces of carbon. As prices rub together dust is created which is no good. Rinse the carbon thoroughly until no dark water comes out before adding it as well

There can be bacteria in the sea water especially if you are experiencing changing tides. The diet as suspected is Inadequate leading to a vitamin deficiency and lack of fats and aminos. That is reason for facial erosion
Add foods such as :
LRS Fish frenzy
Rods original formula
Krill or chopped krill
Marine cuisine

Add selcon vitamins to the foods occasionally also

I actually feed krill too, also would get live crab from sea too plus cockles and muscles , I will get some of the other things u have said

IMHO - at least this fish has HLLE - whether it has another tumor/infection/etc on its nose - IDK - But totally agree - if it's a benign 'tumor' - i.e. growth - that can also be viral - and iMHO - Goes along with HLLE. I.e. A virus. (poor water quality/nutrition/etc) -=

So - the wrasse has two issues. It has HLLE, those are the pits around the eyes. The use of soft, dusty carbon can cause that. Well rinsed pelleted carbon, not in a reactor is the best way to go if you want to use carbon. Once the lesions start though, they rarely heal.

The second issue are the bumps. Male bird wrasses get those in captivity, they can get really large. Nobody knows for sure what causes them, but my suspicion is a build up of male hormones in the water....sort of "super charging" their secondary male sexual characteristics. I've never had this issue go away either, sorry!
