Black face on regal blue tang and possible eye problem

Our 2" regal blue tang has a black center stripe on it's face instead of the normal blue color. Also concerned it's eye doesn't look right.

All other indications including feeding seem normal.
All the other fish in the tank seem normal.
We have had the fish for 5 months before this. Other than an initial ich outbreak when it was first brought home everything seemed ok.

There should be a thin, mostly verticle, black line on either side of the fishes face, running thru each eye.
If you look at it head on like in the picture, the center verticle nose area should be the same blue as most of the body.

Sorry for the lousy pic. It doesnt like the camera much. Just purple seaweed...

The eye issue is barely visible. So far it just seems like a slight enlargement of the clear area of the eyeball.
It is definately larger than the other eye.

Off work today and was enjoying the tank, then noticed the tang looked different.

I think some of them have faces that are all black like that. Here is a couple iPhone pics of my yellow belly blue regal, but my iPhone struggles to get the yellow belly. Mine is about 4 inches maybe a little bigger.
