Any updated photos? Like stated above, antibiotics are slow-acting in fish. It’s only been a few days, you want to treat for at least 10... or as long as 14 if he tolerates it.

Maybe try some extra-enticing foods or adding garlic-guard just to get him through the treatment. Medications can suppress appetite.

Comparing your original pics it seems to have improved some, or is that just the pics?

What are you using for aeration? Antibiotics can deplete O2 quickly. When you did the last FW dip did that seem to provide relief? How long did you do it for? Exactly what meds are in the tank now?
 
I have just the HOB for aeration. I didn’t notice any difference with the dip, nothing came off of him. The dip was 5 minutes. This new QT has nothing in it but NFG for 5 days
 
A powerhead aimed at the surface, and an airstone would help immensely. I don’t believe that a filter alone will cut it with medications. I used all three in my 20g QT and my fish were still breathing a little faster than normal during treatments...

I don’t think he looks terrible though, I know it’s hard but I would just feed him small amounts as often as you can and stay the course for now...
 
Air stone will be added when I get home. So the consensus is he’s O2 deprived. Thanks for the help. Anything for the loss of appetite?

With fish everything is relative, lack of O2 could result in appetite suppression. With these guys you have to go step by step until you finally find the problem. I think more O2 exchange will definitely help.
 
You could try a few different things and see if something perks his interest. Red algae maybe instead of Nori? Fresh clam (frozen first to kill pathogens)? If he’s eating at least the blackworms... he should probably be ok to get through the treatment, and hopefully will perk back up once he is in a med-free tank and feeling better.
