Blotchy skin on Achillies Tang

I'm noticing a few blotchy white patches on my Achillies. He's been in the tank for almost 2 1/2 weeks. All fish were 3 weeks in cupermine copper and another 4 days in formalin. He also occasionally turns gray for a few seconds as he darts across the tank. Could this just be a stress issue? I wish I had better pictures, but he is fast, and darts behind the rocks when I approach the tank.

Achilles can turn grey - that's fairly normal for them sometimes.

Blotchy, though . . .

Honestly, that doesn't sound like ich - velvet might be a possibility. Things can go wrong in QT - copper can drop below therapeutic levels for a few moments, and that can be all it takes.

Brooklynella also seems to be making the rounds, and though it's most frequently associated with clownfish, tangs can get it. That would be treated with an Acriflavin bath, followed up by metronidazole. (I should think the formalin would have taken it out, but ...)

Is he displaying any behavioral symptoms? Heavy breathing, swimming into the flow of a powerhead, hiding from the lights?

He swims up to one power head for a few seconds, almost like he's "surfing" then heads back the other direction. I didn't give that too much thought because I thought they like a lot of flow, and it isn't consistent. He will also on occasion dip to the side when he's swimming and graze the bottom of the tank, but never against the rocks...
Breathing fine, eating well.

I think it's turning out to be flukes. Just dosed PraziPro and he started to freak out. I tried to do a fresh water dip, but this is in a 300 gallon tank, and there are tons of hiding places.
 
Those do look like flukes on him. Photos below for comparison purposes.
