My blue hippo tang starting flashing and I attempted to dose with metaflex after talking with FS. The next morning I notice it was having trouble swimming. Shortly after it died. Looking for help on what people think it could have been. Water parameters are in picture below. All other fish seem fine. Thanks for the feedback. Trying to learn how to prevent this again.I had him for about month and was eating appear to be healthy until the last couple days.

Did you add metroplex or melafix?
Are there any other fish or invertebrates in the tank? How do they look?

Given the limited clues and the photo, I would say this fish had flukes, a skin parasite.

Its a spin test by API and as reliable as test kits

Im not sold on test results. When you get an opportunity, take a water sample to a store that does NOT use Api kits and compare readings- then you'll know where water is at. This is not likely a water quality issue but rather from skin, maybe bacterial but hard to tell from a carcass.
What odd behaviors did you notice prior to loss such as heavy breathing, scratching, loss of color, loss of appetite, etc?
 
It’s an api spin test .
after seeing all the levels tested are extremely low if question salinity and if it’s purposely low perhaps fish only system or salt mix ?

to calculate alkalinity from ppm to dkh multiply ppm x 0.056 to get dkh

Loss of color , flashing /scratching and last day did not eat. Thanks for the reply

Melafix is a tonic, not really a full medication. It won’t fully treat flukes, those require the use of praziquantel.
The symptoms you reported do sound like it had flukes.

As suspected- Fluke symptoms. Melafix as Jay indicated is rendered useless in marine environment and made from Teak Tree oil. For Prazi which Jay mentioned, treatment of choice is PraziPro and while reef safe I prefer QT tank for treating but not necessary. If in dislay tank, remove your skimmer cup but leave the skimmer running as prazi-pro will make your skimmer go nuts. It will produce a truly amazing amount of bubbles so still have your skimmer running to keep the oxygen levels high in the water as some medications like praz-pro can lower the amount of oxygen in the water which is something you want to avoid so I recommend running an airstone in addition to treatment.
Also . . . remove any type of carbon that you have running to allow Prazi to run at full strength which will be 8 day interval, change water and treat again for one more 8 day interval.
