my blue tang has been acting weird, laying on her side. not active like usual. and these spots in her. any idea what it is?

Large, discreet-but-diffuse patches like that suggest a bacterial infection to me - take a read through this protocol for infections:

Something makes me want to say that Brooklynella is also a possibility, though that usually affects clownfish. In any case, you're going to want to set up a QT.

If you can, get them into a QT/HT and treat with Metroplex, Kanoplex and Furan-2. That is your best bet IMO.

Beautiful fish but they will let you know if you don't properly QT everything or if you have water quality issues. They probably aren't more problematic than many other similar Tangs, but they are more popular which makes hearing of problems more common.

He probably scraped himself or something and developed an infection

5 years is a decent amount of time, any other new additions recently? Fish or coral? Some of those spots look parasite-like, and it's very common for a bacterial infection to come hand in hand with parasites like ich, brook, and velvet.
