I can’t tell for sure by the picture, but it almost looks like a flatworm or maybe it just picked something up? Have you tried to take the urchin out and get it off?

Weird... I don’t have much experience with them, I only had one for a year or so and he kept picking up all my corals so sold him back lol, but hopefully someone else chimes in. I would just keep a close eye on him and hopefully it just goes away on its own.
