Blueline angel with small white patch under mouth???

Any ideas what this is? Pics aren’t great. He does scratch the side of his face. He has been in hospital tank for almost 4 months trying to get him right for DT!!! Otherwise he is clean! I have treated him with works!! GC, prazipro, copper,hypo and dips. No meds in water for over a week! But still scratches side of face!! Ugh Doesn’t eat great either. Any ideas would be appreciated. Thanks

Ok thanks, someone mentioned an infection? He has been scratching at his mouth?
 
Hard to tell from pics, but he has been hammered by medical treatments so is susceptible to outbreaks of many things. Double down on water quality in treatment tank. Lympho usually reveals itself at the edges or extremities of the fish, these typically have little blood circulation thus lower immune response due to it.
 
She is very thin. I agree that unless you see something imminently life threatening, I would hold off on any more meds, keep water quality pristine (how big is the QT? How often/much do you change water?) and focus on getting her fatter. What does she actually eat? How often are you feeding? I recently had a flagfin go through QT. Very thin when she arrived, and only ate 3-4 small Formula 2 pellets at a time. It can take some time....
 
Hi, she is in 65gal tank plus sumps. Skittish for months when i first got her!! Would hide when go near tank!!! Much better now. Will only eat at night. Clams on half shell, you name it would be partially eaten the next morning. Had a bad case of flukes which were hard to eradicate!! After all treatments still scratched her face. Went at lesat a total of about 1 1/2 without any treatment to see how she would do but almost every time i put frozen food in water she would scratch her face and swim away!!!! With no visible signs of parasites. Now i saw this so it make me wonder if she had a infection in her mouth all this time?? Never used antibiotics on her. Really at a loss. But it would kinda make sense??? I dont know this has been the toughest fish i QTED in years !!! Really frustating!! What do u think??
 
Its a tough one, stressed from treatments, immune may be low due to food intake. Angels are very sensitive and shy in some species and susceptible due to it.
Hmm, i dont know but antidepressant methylene blue bath maybe, but i think water quality and concentrate on getting high nutrient foods into her will help, thus my suggestion for selco or simmilar.
 
Ok thanks! Yeah adult Bluelines are normally shy but this one took it to the extreme. I appreciate your help!! Any Mods want to chime in???
 
I am most concerned about the eating situation. Angels are diurnal, so I don't really understand "eats at night". I also really don't like the idea that you don't actually see her eating. Have you tried just ambient room light instead of using the tank light? Is it just that she won't eat in front of you? That's a pretty big QT.... not as easy/convenient to do water changes on. Are you sure water quality is good (especially considering you are leaving opened clams in the tank for extended periods)? Have you tried live brine shrimp or live blackworms to get her used to eating out of the water column?
