Started out of nowhere two days ago on my two candy canes. Previously they were doing fine. Within two days the top one appears to be melting and has exposed skeleton everywhere.

Did a water change last Sunday with a new salt, but tested my parameters and they had not changed other than nitrates/phosphates being lowered. Calcium 380, alk 12dkh, ph 8.3, salinity 1.026, nitrate 0, phosphate 0.25.

0 nitrate is bad. I assume you have the API test kit but if not, 0.25 phosphate and 0ppm nitrate is a bad combo for coral health. This also is worsened by high alk as that requires they have enough nitrate.
 
As stated above. When your parameters are off for an extended period it is bad for corals and the stress can cause BJD to become active from a dormant state. In my tank it started with 3 heads on a duncan but spread quickly to torches and a hammer also.
