Bubble tip dying?

i have had this bubble tip going on 2 weeks now. I don’t know too much about them but it seems as though the health of it is slowly deteriorating. It’s foot is still attached to the rock pretty securely but the color is straying to turn brown and some of its tentacles are becoming tangeled and losing its white tips.

Ya I have a 30 gallon been up and running for 6 months. Ammonia 0, nitrite 0, ph 8.4, nitrate 5 ppm and salinity 1.025. I have a t8 light setup with the coralife 50/50 bulb. Just one

Your lighting isn’t sufficient enough for nems. You’ll either need to upgrade or part with it
 
I put a xr15 on my 29 anemone cube and absolutely love it. It is a little more than 300 but totally worth it imo

It’s going to look bleach until you upgrade. I would start weekly feedings
 
From your pics it doesn't look like at BTA, looks more like a white Sebae.
