Kinda blurry but it appears to be flat worms. Several ways to deal with them. If you go the route of medicine try to remove as many first as possible and keep up with water changes.

Very tedious work! Small tube and syphon them out one by one. You can try a wrasse too and get a small powerhead that will blow them into the water column out of the cracks of the rock and corals some fish will munch on them from the water. Not fun but easily manageable with some work.
