Can Anyone ID This LPS?

My daughter fell in love with this little guy so I picked this one up for pretty cheap at my LFS. They were not sure as to the type, but it is an LPS and will actively take in food when target feeding. It is looking rather ragged, and I want to find out what it is in case it doesn't make it so I can get another one since she is totally enamored with it.

It doesn't have any sweepers or feeding tentacles that I have seen and it is attached to a skeletal base, and stays in this dished shape, even when puffed up. The notches are where it got nipped by a blenny.
